Abstract: The treatment of Chronic Obstructive Pulmonary Disease (COPD) is difficult due to large variation in clinical presentations, underreported symptoms, frequent comorbidities, disappointing effects of medical treatment and problems with giving up smoking. Treatment is focused on care rather than cure, and consists on improving quality of life and daily functioning of patients. In this thesis we explored different aspects of optimal COPD care, including optimal medical treatment and self-management. More specifically, the primary aim was to study the potential role of integrated disease management (IDM) programs for COPD, specifically in primary care
